Test cases for database (#16)

* fix: remove unused import

* fix: use env instead of hard code for create database instance

* refactor: change most "all" hook to "each" hook

* refactor: split test suits into files

* add: use DB_LOG_SQL in env to control SQL in log

* add: test for logical operator and fixes
